(1) What is the form of this sacrament?

(2) Whether the form for the consecration of the bread is appropriate?

(3) Whether the form for the consecration of the blood is appropriate?

(4) Of the power of each form?

(5) Of the truth of the expression?

(6) Of the comparison of the one form with the other?
